Proxy Access
Need to manage a family member's health information? MyChart proxy access allows you to view and manage records for your children, elderly parents, or other dependents.
To set up proxy access, please visit the front desk during your next appointment or contact the clinic by calling and leaving a voicemail. You will need to complete an authorization form. Please allow up to 24 hours for a return call.

MyChart is a free, secure online patient portal that gives you convenient access to your HAVEN Free Clinic health information. You can use it from any device with an internet connection to communicate with your care team, view test results, request appointments, and more.
To create your MyChart account, you need an activation code. You can get one from your after-visit summary, during your next appointment, or by requesting one at the front desk. Your activation code is used once to log in for the first time, after which you will create a unique username and password.
Yes. MyChart is a completely free service offered to all HAVEN Free Clinic patients. There are no fees or charges to create or use your account.
Yes. The MyChart mobile app is available for both Apple and Android devices. You can download it from the App Store or Google Play. You can also access MyChart through any web browser on your phone or tablet.
After logging in, navigate to the Medications section and select the prescription you would like to renew. Follow the prompts to submit your renewal request. Your care team will review and respond to your request, typically within 2–3 business days.
Yes. MyChart offers proxy access, which allows you to view and manage the health information of a family member or dependent. To set up proxy access, ask at the front desk during your next visit or contact the clinic directly.
Absolutely. MyChart uses industry-standard encryption and security protocols to protect your personal health information. All communications within the portal are private and attached to your account only.
